Information necessary.
1) An explanation of the allegedly violated intellectual property rights.
2) A description or website link for the item you allege is being violated.
3) A list of your contact details, such as your home address, cell phone number, and email address.
4) A declaration from you, made under oath, confirming the accuracy of the information in your notice and your ownership of the IP rights or your authority to act on their behalf.
